Application for Library Cards Policy

1. All persons must have a library card to take books or materials from the library. Signing a library card validates all loan privileges and responsibilities of library use. Three year permanent and one to six month temporary cards may be issued.

2. It is the intent of the Mohave County Library District to provide library cards (and thereby bestow the attending privileges and responsibilities of library use) to all persons who can show proof of name and current mailing address (General delivery addresses are acceptable), and are willing to comply with the rules and regulations for use established by the Library District. The qualifying documentation listed below serve as guidelines for the issuance of cards.

3. All applicants must apply in person for a library card.

a. Adult status library cards (18 years or older) will expire three years after date of issue.
b. Young adults between the ages of 16 and 17 (inclusive) years will be issued a library card without the need for a parent's or guardian's or responsible party's name if they meet all requirements for an adult status library card as listed in item four below.
c. All children under the age of 16 and young adults between the ages of 16 and 17 who do not qualify for an adult card, must obtain the name and proper documentation of their parent/legal guardian/responsible party before being issued a Child status library card. Parent/legal guardian/responsible party must meet borrower requirements. The parent/legal guardian/responsible party is encouraged to be present at the time of approval of application for the library card. When the parent/legal guardian/responsible party can not be present, the child applicant can present a completed form and an item as listed in item 4 below verifying the parent or legal guardian's mailing address. The purpose of the parent/legal guardian/responsible party's presence and signature is to provide notification that the Library is not responsible for monitoring materials checked out by children and that the adult is financially responsible for lost, damaged, or unreturned items. It is the intent of the Library to encourage and provide opportunities for the parent/legal guardian/responsible party to be fully informed about the policies and procedures of the Library pertaining to the issuance of borrower cards to their child and the use of the Library by minors. This intent may, in a small minority of cases, be detrimental to the ability of a child to obtain a borrower's card. When library staff has made a good faith effort to secure the parent/legal guardian/responsible party's involvement in the issuance of a borrower's card to a minor, a card will be issued even if staff cannot verify with absolute certainty that the parent/legal guardian/responsible party has been informed. Child status library cards will expire three year after date of issue or on their 18 birthday.
d. Emancipated children (under age 18) must meet all requirements for an adult library card.

4. Applicants for a three year permanent library card must fill out a library user information sheet and show any of the following information to prove name and current local mailing address.

a. A valid drivers license, current vehicle registration, or current vehicle insurance card.
b. An Arizona Identification Card issued by the Motor Vehicle Division of the Arizona Department of Transportation.
c. A military identification card showing current active service.
d. A current utility bill (current or previous month). The bill may be telephone, water, gas or electric.
e. A rent receipt or other local business receipt showing name and mailing address (current or previous month).
f. An imprinted personalized bank check or business card.
g. A voter's registration card or Department of Economic Security (DES) card.
h. A piece of mail with the applicant's name and current mailing address postmarked for the current or previous month.
i. A current report card from a Mohave County junior high, high school, or community college.
j. Other appropriate material at the discretion of the Librarian/Station Coordinator.
k. Verification of a parent/guardian/responsible adult address through existing borrower records.

5. All approved borrowers will be assigned an I.D. number and an alternate I.D. number. Applicants for library cards will be asked to provide their Social Security Number as an alternate I.D. number, but no applicant will be denied a card based on their refusal to provide their Social Security Number.

6. All applicants for a borrower's card will be asked to provide their month, day, and year of birth, but no applicant will be denied a card based on their refusal to provide their year of birth.

7. Non-Mohave County permanent residents of nearby communities or nonresident Mohave County property owners may apply for a three year permanent library card. They must provide proof of name and current mailing address as listed in 4 above based on the state in which they reside.

8. Applicants may choose to apply for a temporary one to six month library card and must fill out a library user information sheet and show:

a. A valid driver's license from their home state.
b. Plus one item listed in 4 above showing a local mailing address.

9. Applications that have been denied are referred to the local librarian in charge and maintained for a period of no less than 60 days. When application for a borrower card is refused, the reason for refusal will be written on the back of the library user information sheet by library staff and the sheet forwarded to the Library Manager. Upon request, applicants denied a library card based on the above criteria will receive written or verbal notification concerning denial of borrowing privileges within a reasonable period of time.
